FAMILY APPLICATION 

Dear Parents, 

The Burbank Coordinating Council would like to offer you the opportunity to participate in our 2023 Holiday Basket Program, which provides food and gifts for you and your family in December. If you qualify for Free or Reduced priced lunches, and you would like a basket for your family, please complete our Google Form on or BEFORE November 24, 2023. (No paper applications will be available. If you require assistance, please contact us at bcc.holidaybaskets@gmail.com).

Baskets will be distributed in December 2023. Information on the location for distribution will be emailed to you in November, once your application has been verified.

 

Please also remember the following:

  • In order to participate, you must apply online before the deadline.

  • Qualify for Free & Reduced Lunch or provide other documentation to show financial need

    • documentation will be required to be submitted digitally 

    • A list of acceptable documents will is on the Google Form Application

  • If not matched with a sponsor, you will be responsible for picking up your family’s basket on December 10th.

  • Have a valid email/ phone number so your family’s sponsor or BCC can contact you- keep your information current

  • Failure to follow these guidelines may result in you not receiving a basket

 

Instructions for completing your application online:

  • Click on the Family Application button and fill out the Google Form.

  • Fill in the information requested on the Google Form. You must fill in ALL fields.

  • If you have special needs or concerns, describe them in the Notes Section of the form.

  • Click submit and you are done.

 

If you don’t have access to a computer, here are some suggestions on how to submit your form:

  • You can use your iPhone or Android phone or use a tablet, iPad, or Chromebook.

  • You can ask a family member or neighbor to assist you or you can visit your local library for online access.

  • You can contact your child’s school for assistance and access to the internet.

 

***We do not sell your information. The information you provide us is used to make sure your family is receiving the correct items and sizes specific to your family's needs. Your contact information will be used by the sponsor who is building your basket or by BCC.

El Consejo de Coordinación de Burbank quisiera ofrecerles la oportunidad para participar en nuestro  Programa de La Canasta Navideña que provee comida y regalos para su familia en diciembre. Si Usted califica para almuerzo gratis o almuerzo reducido en precio y quisiera recibir una canasta navideña, llene el formulario de Google en nuestro sitio electrónico antes del 24 de noviembre. (Formularios en papel no estarán disponibles. Si requiere ayuda, contactenos en bcc.holidaybaskets@gmail.com).

 

Las canastas serán distribuidas en diciembre de 2023. Le avisaremos por correo electrónico en noviembre cuando su aplicación sea verificada para decirle a dónde se podrá recoger la canasta.

Por favor tome lo siguiente en cuenta antes de llenar el formulario: 

  • Debe aplicar electrónicamente antes de la fecha de límite

  • Debe calificar para Almuerzo gratis o Reducido u proveer otro documento que demuestra necesidad financiera 

    • poder entregar el documento digitalmente

    • una lista de documentos aceptable estará en el formulario de Google

  • Su correo electrónico y teléfono celular deben ser válidos, y Usted debe avisarnos si cambia su información

  • Un adulto de su hogar debe estar disponible para recoger su canasta si no está emparejado con un patrocinador 

  • Si no sigue estos pasos, tiene el riesgo de no recibir una canasta 

 

Instrucciones para completar su aplicación electrónica- este es el método preferido:

  • Visite el enlace www.burbankcc.org/familyapplication. Haga clic en el botón Family Application

  • Llene la información en el formulario de Google. Debe llenar todas las secciones.

  • Si usted tiene alguna situación especial, habrá una sección en el formulario para avisarnos.

  • Haga clic para entregar el formulario y lo ha logrado.


 

Si no tiene acceso al internet, aquí hay algunas sugerencias en cómo entregar el formulario digitalmente:

 

  • Puede usar su teléfono iPhone or un app en su teléfono celular Android o puede usar una iPad o un Chromebook

  • Puede pedirle ayuda a un familiar, un vecino, su hijo/a, la escuela de su hijo/a o puede visitar la biblioteca también

 

No vendemos su información. La información que usted provee se usa para asegurar que su familia está recibiendo los regalos y los tamaños de ropa correcta. Su información de contacto se compartirá con su patrocinador que le proveerá su canasta o con BCC.

Բրբենքի կազմակերպչական խորհուրդը (BCC) ցանկանում է հնարավորություն տալ ձեզ մասնակցել 2023 թվականի տոնական զամբյուղներ ծրագրին, որը նպարեղեն և նվերներ է հատկացնում ընտանիքներին դեկտեմբեր ամսվա ընթացքում: Եթե դուք անվճար կամ զեղչված գնով սնունդ եք ստանում դպրոցում և ցանկանում եք զաբմյուղ ստանալ ձեր ընտանիքի համար, խնդրում ենք լրացներ Գուգլ ձևաթուղթը ՄԻՆՉԵՎ 2022 թվականի նոյեմբերի 21-ը: (Թղթի վրա տպված դիմումնագրերն առկա չեն: Եթե ձեզ օգնություն է հարկավոր, խնդրում ենք դիմել bcc.holidaybaskets@gmail.com հասցեով):

 

Զամբյուղները պատրաստ կլինեն շաբաթ, 2023 թվականի դեկտեմբերի 10-ին, առավոտյան ժամը 9-ից 12-ը:  Ձեր դիմումնագիրը վերանայվելուց հետո, նոյեմբեր ամսին ձեզ կտեղեկացնենք, թե որտեղից վերցնեք ձեզ հասանելիք զամբյուղը: 

Խնդրում ենք նշել հետևյալը. 

  • Մասնակցելու համար, դուք պետք է դիմումնագիր լրացնեք մինչև նշված ժամկետը: 

  • Դուք պետք է որակավորվեք դպրոցում անվճար և զեղչված սնունդ ստանալու համար կամ հատկացնեք այլ փաստաթուղթ, որ դուք ֆինանսական օգնության կարիք ունեք:
